OxyContin is one of the most widely prescribed — and most widely discussed — opioid pain medications in the United States. Since its approval by the FDA in 1995, it has helped millions of patients manage severe, chronic pain while also becoming central to conversations about the opioid epidemic. Understanding what OxyContin is, how it works, when it is medically appropriate, and what risks it carries is essential for patients, caregivers, and healthcare professionals alike.
What Is OxyContin?
OxyContin is the brand name for oxycodone hydrochloride extended-release (ER) tablets, manufactured by Purdue Pharma. The active ingredient, oxycodone, is a semi-synthetic opioid derived from thebaine, an alkaloid found in the opium poppy plant. The "extended-release" formulation is the defining feature of OxyContin — unlike immediate-release oxycodone, which acts quickly and lasts 4 to 6 hours, OxyContin is designed to release medication slowly into the bloodstream over 12 hours.
OxyContin tablets are available in multiple strengths: 10 mg, 15 mg, 20 mg, 30 mg, 40 mg, 60 mg, and 80 mg. The 60 mg and 80 mg tablets are reserved for patients who are opioid-tolerant, meaning their bodies have adapted to regular opioid use. The 80 mg tablet is among the highest-dose oral opioid formulations available commercially.
In 2010, Purdue Pharma reformulated OxyContin with abuse-deterrent technology, making it harder to crush, snort, or dissolve. This reformulation has been associated with a reduction in certain patterns of misuse, though it did not eliminate the risks of addiction or overdose when taken orally as intended.
How Does OxyContin Work?
OxyContin works by binding to mu-opioid receptors in the brain, spinal cord, and peripheral tissues. When oxycodone activates these receptors, it reduces the perception of pain, decreases the emotional response to pain, and produces sedation. It also activates reward pathways in the brain, which is the primary driver of its addiction potential.

Opioid withdrawal is rarely life-threatening in otherwise healthy adults, but it is extremely uncomfortable. OxyContin should never be stopped abruptly — dose tapering under medical supervision is essential.
Drug Interactions
OxyContin has clinically significant interactions with numerous medications and substances:
- Benzodiazepines (Xanax, Valium, Klonopin): This combination is responsible for a large proportion of opioid overdose deaths. The FDA mandates a black box warning for this combination.
- Alcohol: Dramatically increases CNS and respiratory depression. Can be fatal.
- Other opioids: Additive respiratory depression risk.
- MAO inhibitors (MAOIs): Contraindicated; can cause severe serotonin syndrome-like reactions.
- CYP3A4 inhibitors (certain antifungals, antibiotics, HIV medications): Can increase oxycodone blood levels and risk of overdose.
- CYP3A4 inducers (rifampin, carbamazepine): Can decrease oxycodone levels, reducing efficacy.
- Muscle relaxants, sleep aids, antihistamines: Additive CNS depression.
Who Should Not Take OxyContin?
OxyContin is contraindicated in patients with:
- Significant respiratory depression or compromised airway
- Acute or severe bronchial asthma without resuscitative equipment
- Known or suspected gastrointestinal obstruction (including paralytic ileus)
- Hypersensitivity to oxycodone or any component of the formulation
- Concurrent use of MAO inhibitors (within 14 days)
OxyContin should be used with extreme caution — or not at all — in elderly or frail patients, patients with compromised pulmonary function, patients with history of substance use disorder, patients with head injuries or raised intracranial pressure, and patients with hepatic or renal impairment.
OxyContin vs Immediate-Release Oxycodone
A common question is the difference between OxyContin and immediate-release (IR) oxycodone products. The key differences are as follows: OxyContin is designed for around-the-clock pain control with 12-hour dosing intervals, while IR oxycodone is used for acute or breakthrough pain with dosing every 4 to 6 hours. OxyContin should not be used as a supplemental dose for breakthrough pain — a separate IR opioid is typically prescribed for that purpose.
Overdose: Signs, Response, and Prevention
OxyContin overdose is a life-threatening emergency. Because each tablet contains a large amount of oxycodone, a single extra dose or accidental ingestion by a child can be fatal. Signs of opioid overdose include extreme drowsiness or unresponsiveness, slow, shallow, or stopped breathing, blue or purple lips and fingertips (cyanosis), pinpoint pupils, gurgling or choking sounds, and limpness.
If you suspect an overdose, call 911 immediately. Administer naloxone (Narcan) if available — it reverses opioid overdose and is available without a prescription at most pharmacies in the United States. The effects of naloxone are temporary, so emergency medical care is still essential even if the person appears to recover.
Prevention strategies include storing OxyContin in a locked, secure location away from children and other household members; never sharing medication; disposing of unused tablets at an authorized medication drop-box; and having naloxone available in the home.
Safe Storage and Disposal of OxyContin
Proper storage and disposal of OxyContin are critical public health measures. Store all oxycontin tablets at room temperature, away from moisture and heat, in a child-resistant container in a locked cabinet. Unused OxyContin should be disposed of promptly — do not keep medication "just in case." The DEA operates authorized collection sites at many pharmacies. If no collection site is available, the FDA-approved drug disposal method is to mix the tablets with an undesirable substance (like used coffee grounds or dirt) in a sealed bag and place in household trash.
